Do they have to be perfectly in line with each other in terms of height?  Or can one be higher or lower than the other by say a half centimeter or so without any problems.

You will want them inline.  After seeing someone else's screwed up alignment, I sprang for the Larue set of mounts.

The magnifier has internal adjustments, so they can be a little off.
