Alright, so I would post pictures of my issue but I honestly do not think it would really help, I am going to just try to explain the situation as it occurred.

I had just shot about thirty rounds and took my Daniel Defense M4 in to clean. Popped the first pin out in the lower receiver and I go to pull the top up and hear this terrible clang. As best as I could tell my retaining pin and come loose, the buffer tube had shot forward inside the rifle up against the trigger and it was all just a mess. The spring became incredibly mangled in the process, and the buffer pin its self is warped now after repeated attempts to try and replicate the problem to see what is going wrong, it wont even fit back in the hole. Long story short, I have ordered a new pin and spring, that is no biggie. My biggest question is why would it have done this to begin with? I recently installed a Magpul ASAP plate, but the pin and everything seemed fine to me. Is the most likely cause that I just did not get the buffer tube over the pin enough, or could something else be wrong?

Post a picture depicting the amount of receiver extension threaded into your receiver. If it is not in enough, the buffer retainer will not be reliably held in place.

Is the retainer hole damaged in the receiver? Is this a fixed or collapsible stock?

Unless the buffer retaining pin hole is horribly deformed, you'll be fine with the new parts.  Just be sure the receiver extension is screwed in enough so that it overlaps the skirt of the buffer retaining pin.

Yep, probably need to screw your buffer tube in one more turn.

Yep, mine flew out too. Only after I had it opened up. One day I may even replace that pin. Shoots fine without it, I just have to remember to hold the buffer in while breaking her open.
