Does anyone make them?

Oly has then, but I have the Colt system, so they aren't an option.

I assume by    "OLY has them"    that you is wanting a 11.5" barrel with 5.5" hider to maintain legal Title-1 status on your firearm ??

If so, call Steve at ADCO, have him cut a 16" and install a 5.5" hider for you...

How do these have to be attached to be considered permanent?  I mean, in addition to the threads...

Weld is the simplest, advise do it with TIG in couple spots to show intent... 2nd is  pin and weld,  bore hole through muzzle device and into barrel, insert a pin to lock the 2 together, and weld over the top.
