Need a simple pouch style knife sheath for a small fixed blade. Either leather or kydex.

Any suggestions?

You can order kydex and leather kits online that are fairly simple to complete. I don't like knife sheaths that hang to my knees so when my current crop of modded sheaths wear out, I am going to get some materials.
