There's a few members here that have personal contact with his wife. I'm sure they would post if anything major had happened. I'm thinking he got pisssed at the iPad and frisbee'd it. |

Well, John, you seem to have made a difference with me, as I am still doing the "find the good in each day" approach. So simple, but it's a practical tune-up for my attitude.
In other news, we can put man on the Moon and things on Mars, but can't seem to design Christmas lights that don't tangle at every opportunity. |

I’ve written this then erased it 4 times now, worrying it might not be a good idea, all things considered. I’ve decided to say to hell with it, its a good example of finding the good, even when things are horrible. Headed home from work this past Friday night at 11pm. Long-time fiance calls and says she’s leaving one job to head to the other hospital she works at to be with her aunt who was just brought in. The aunt is 54 and has significant cognitive impairment, roughly equivalent to a 4 year old. The fiance grew up with and cared for the aunt. Aunt is super happy, super sweet, and has no medically concerning habits or conditions aside from being a bit plump. I arrive just as the doctor finished trying to get the woman to understand that she is filled with tumors and that he has no way to fix her. She is absolutely hysterical and unconsolable. Thank God good docs still exist, as we expressed our concerns about her now having a heart rate over 180 due to her panic. We immediately put her into peaceful oblivion with ativan, to have a chance to make plans. As required, he explained all the options, including aggressive chemo, some poured directly into the abdomen. Thankfully, the family understood that this would be torturous for her, and immediately opted for comfort care only. The doc agreed that she was unable to process that kind of news and that the psychological torture she would undergo would far exceed the physical pain she was experiencing. Her life expectancy ways mere days. Along with the nursing staff, we decided we would shoot for that kind of twilight state where she would be mostly asleep, but able to be roused to see her family and friends. The doc wrote the most open ended orders for morphine and ativan that I’ve ever seen in order to allow nursing to constantly tweak her dosing. For a day and a half, we had a continual parade of family come through, along with her long-time staff from the day program she attended. Some had been with her for over a decade. Nursing had given us a double room and removed the second bed to accommodate us. On Sunday morning we got a call that many of her friends from the day program and attached residential program would like to come say goodbye. Oh boy, that wasn’t going to be easy, but nursing coordinated a meeting room so program staff could meet with and prepare the clients. As expected, most were very upset, but amazingly, every one of them was able to walk into the room wearing a smile. They all got to tell auntie that they loved her, and she said the same back. They told stories and laughed. It was amazing and beautiful! Not one single tear, not one slip of the tongue about her condition. About one hour after the parade of friends ended, auntie began to struggle with her respirations and blood/lymph flow to and from her head/brain was being restricted. My fiance and I explained the situation to the family present. We told them auntie was losing oxygen to the brain and that she would soon be beyond any more consciousness. We all got to say our last goodbyes before she lost consciousness. We brought up the ativan dosing just a hair to make sure she didn’t regain consciousness and panic. Within a few hours, she was peacefully gone. I can’t tell you how many times my heart felt ripped from my chest over that 48 hour period. But there was an absolute metric shit ton of good too. It was good that she was brought to the small, ancient catholic hospital that still has great nurse/patient ratios. It was good that the attending doc not only understood the difficulties of a terminally ill person with significant cognitive impairment, but also was willing to ask advice from a psych RN that doesn’t even work at his facility (me). It was good that we were able to get her friends in to see her, she was never happier than when she was with them. It was good that the entire family was on board with the game plan. It was good that, aside from her initial 5 minutes of panic, she felt safe, loved, and had her whole family with her. And it was good that I was able to explain to the family after it was done, all the good they had been able to create for auntie at her end. Thank you so much, John. I don’t think I could have found a shred of good in this past weekend without your insights. You are forever a part of me now and you have given much good to a horrified family that you have never heard of. May the good you have led us to come back to you a thousand times over.
