How much difference does it really make?

Does one really need much variation in the width of hand placement?

Should one just stick with the placement that is the strongest? Weakest?

I don't do pushups so no idea there but on my pull ups and pushdowns I just enjoy changing up grips/grip width from time to time.  No idea if it matters, I just like changing it up.

ETA- I will say on my pullups specifically: I could only do them with palms facing each other for a long time, after I could get to 10-12 reps did I have the strength to change up my grip on those.

Whatever grip doesn't hurt is the right one for you.

Changing the width and angle of your hands while doing pushups enables you to engage different muscles and eke out a few more reps if you are going to failure or taking a PT test.
