Speaking as a surgeon who's in the middle of his career....

Obamacare is just one of a multi-factorial reason doctors are leaving medicine. The .gov has decided it's too expensive to pay doctors--and that PA's, nurse practitioners can do just as good a job, cheaper. This is especially true if these "physician extenders" follow predetermined, cost effective algorithms & use an electronic medical record(EMR) which has turned doctors & nurses into data entry clerks, preoccupied with completing .gov-mandated EMR to avoid fines, penalties & negative internet reviews.

I just had this conversation with a nurse practioner. Even tho' she doesn't have the 9 years of training a surgeon has--she's on a committee to direct patient care. She feels her 4 yrs of training qualifies her to tell doctors ( minimum of 7 yrs of training) or surgeons ( minimum of 9 yrs of training)  how to do their jobs. If she feels a patients needs surgery--a surgeon should operate--even if the surgeon disagrees.makes any sense to you? Me either.

And hospital administrators buy into this, through the .gov.....

So now that fewer people want to go into medical school, less bright folks are getting accepted, which affects the "final product" unleashed on the public. Some of these folks fail boards multiple times & are still allowed to practice.

What do you call the last person in the medical school class?

"Doctor."

So before anyone blames Obamacare as the sole reasons doctors leave the profession--bear in mind it's one of several reasons.

The example you point out, though, it why it's wrong to have a government controlled/reimbursed health care system.  The hospital administrators only care about making money and direct every effort to that end.  Despite their pleas that "we're doing this for the patients", their decisions are 100% solely to increase revenue, even if that means 'encouraging' certain surgeries over others.  It's absolutely disgusting, but we docs lost control in the hospital over the last 30 years and this is the consequence.  The government tells the hospitals how to get reimbursed, and the hospital jumps.  Then the directives come down to us.

Our family doctor retired recently and sold the practice to the local hospital.
He was pretty close to retiring anyway but the government aggravation just moved things up.

First, the percentage of doctors leaving Medicare each year is miniscule, and, as I already wrote, is smaller than the number of new doctors accepting Medicare.

In a new briefing issued this week by the U.S. Department of Health and Human Services, the federal government said the percentage of all “office-based physicians” accepting Medicare patients hasn’t changed significantly between 2005 and 2012.


Second, the quality of care only suffers if they decide they want to make the same amount of money and therefore have to see more patients in the same amount of time if they continue to accept Medicare.  What other conclusion could someone come to other than the doctor is not willing to work the same for less money?

Third, the percentage of these doctors is still small enough (< 25% don't accept Medicare currently and only 1 to 2% stop accepting Medicare each year) that it doesn't affect a patient's ability to find a doctor (What will have an impact on that is Obamacare made no provisions to increase the number of doctors/PA's/APRN's to accommodate the 30+ million new American patients and the 12 million new Mexican-Americans).   However, if enough doctors jump ship then the government will make sure that every doctor has to see every patient.

My wife is office manager for the only ENT left in the are,the others (3) sold their practices to the local hospitals and 2 of them have now left the area,the Dr my wife works for is has been talking with one of the hospitals but nothing has come of it yet and he has also talked about a cash only business.



we have also lost the only urologist/proctologists in the area

I had to switch doctors in August because my regular doctor pulled the plug on his practice and went cash only...He is only 45. Couldn't take the bullshit from the Government on top of all the other challenges that come with running a solo Family Practice.

I have the introductory visit with my new doctor and tell him only to find out he is considering the same thing and then says hundreds of thousands of doctors are going to quit in the next three years.

Not at all.  I'm describing it from the perspective of the doctor who's leaving:  "I'm not going to make as much money so I'm going to take my ball and go home because making less money for the same amount of work is a more important factor than taking care of patients."  We're talking about 1 to 2% of doctors doing this per year.  In communist countries doctors, teachers, cops, etc all make the same amount of money.  So why are there still doctors?  Because the vast majority of people don't go to medical school because of money.  

You just don't get it:  We doctors are being paid with dollars borrowed from your children and grandchildren.  We can either make cuts now or we can watch all savings become worthless.  No I most certainly do not have any compassion for the 1 to 2% of doctors that leave.

I provide IT support for area small businesses.  And I can't tell you how many medical clients are gone because of ACA.  Any of the small practices that haven't been gobbled up by the conglomerates with providers aged 50 or slightly over just simply packed up and retired early.  Just in our family, we've lost 3 doctors that simply said "Im out."

They were already teetering with reduced payments from medicare/medicaid and increasing malpractice insurance premiums.  ACA, EHR, etc just pushed them over the edge.  

Normally Im one of those "meh, survive or die in the marketplace" type of guys but these were good doctors that just simply had enough of the BS.    And finding a good doc that you trust, etc. isn't an easy thing to find.

Look for this to happen on a larger scale with regional hospital providers. Locally a regional provider projected a possible future where they closed due to the ACA. They figure it will be 50/50 on whether they will remain  a stand alone system or be consumed by one of the larger entities like JHU or UPENN.

I was misdiagnosed on a torn ligament for months by the PA. I finally insisted on seeig the doctor and I was correctly diagnosed within 5 minutes. Never trust the PA to get it right. They aren't doctors and shouldn't be employed as such.

Yes.  No, specialist.

Adult primary care became awesome (ETA: from a lifestyle standpoint) though in the last 5 years.  One of the very difficult aspects of primary care was having to take call.  Now that the country has moved to a hospitalist system where all in-patients are taken care of by hospitalists who don't see outpatients.  The internists had a drop in income as a result, but it's been somewhat offset by weekend same-day appointments, and less interruptions during the weekday so they can be more efficient seeing patients.  I'm still doing call which has its advantages.

That's currently not true, but it is what the government wants to incorporate into reimbursement ie. patient satisfaction.  In an ideal world, there would be no insurance except for catastrophic events, and mediocre doctors would make less money because less people would see them and they would get less referrals from other doctors if they were a specialist.  Then there would be an incentive to change the way they practice.

While I do not like the way health care is going the truth of the matter is the fact that these doctors can retire is part of the problem.  The doctors I represent who are retiring are not taking other jobs, they are just retiring.  And that means they have been made so much in 10 or 20 years of practice that they can stop working.  This is in fact a part of the problem.

If the doctors were retiring to go sell real estate than we would have a different problem, but the current problem is a sign of one of the many things wrong with health care.

Her debt is/will be 4 years.




She's in the Health Professions Scholarship Program (available to all branches, she's in the USAF).  The Air Force is paying for her med school and paying her a monthly stipend while she's in med school.  When she finishes her residency she'll owe 4 years.  She's currently a 2LT and when she completes med school she'll get recommissioned as a CPT.




It's an amazing deal.

10 years is fantasy.  20 years?  Maybe if they were a high-powered neurosurgeon or cardio-thoracic surgeon, they don't have children, the wife still works and can carry insurance for them, and they live modestly, then sure.

That's nothing new. For some years my gynie and internist were a wife (gynie) and husband team from India that had their own practice. They had been here for maybe 30 years. English very understandable.

Now, I go to a group practice that's attached to one of the urgent care centers run by a local hospital. I'm seen by one doctor who is a younger woman and I have no issues. She's a GP, which is what I prefer.

My sister is a dermatologist.  She bathes in money.  She has an armored truck back up to her clinic at the end of the day and she shovels money into the back of it!  My dad actually made fun of her when she chose to do that.  She has ZERO call and an endless amount of patients.  She also does a public service by removing tattoos.

Right now, you'd be hard-pressed to find something better in terms of lifestyle and low stress than adult primary care.  Work 4 to 4 1/2 days a week.  No call.  Every weekend off.  You can build long-term relationships with patients.  They're also going to be in very high demand with the influx of patients and illegal-to-legal immigrants.

I got a buddy that I ride bicycles with. He sells artificial hips and such. He makes 5-8X what the guys installing them makes. All with a four year degree from a state school. If money's what your after there are better paying non hands on jobs in the medical field.

Whats old is new again.  Under Batista, Cuba had a booming medical tourism trade.  Med care every bit as good as the USA at a fraction of the price.  Abortion on demand was popular there for those "in trouble" and with the cash.



If they wanted to, Cuba could become a world class medical destination again in less than a generation.  Free market and money has a way of filling any void.

We both chose poorly.  Remember that they are doing procedures all day of one sort or another.  Yes, they do sometimes have to spend 30 minutes going over various acne therapies with a patient, but most of what they do is cutting something off and sending it to pathology.  Some actually do a derm path fellowship and read their own slides.  There are a myriad of cosmetic stuff you can get into like Botox (I do Botox but only for spasticity, blepharospasm/hemifacial spasm, etc).

That's all going to change now.  With Obamacare, the "system" needs to have doctors practicing so jobs will be more secure.  There is a lot of flux now with docs trying this or that type of practice, but in a few short years when the music stops playing, there will only be one Obamacare chair left to sit in.  This isn't all bad.  With the advent of hospitalists, the lifestyle of adult primary care has made a 180 degree turn.

Not if you believe all that stuff about medicine being a calling and not an occupation.  

Personally I think emergency medicine is a supremely useful skillset to have.  But I can imagine that would be of little comfort if work sucks every day.
