I'm adding the title 1 LMT receiver mount to the line item on my form1 submission. I thought I saw an archived thread from a year ago on the very same questions I have below but I am unable to find it.

Should the Manufacturer field read "LMT Defense" or what's engraved on the receiver instead, which is "Lewis Machine & Tool Co."? I see there's already a verified "LMT Defense" in the selection list on the ATF form site.
For the Model field, should this read "M203" or "M 203" with a space which is how the receiver stamping reads? Or should it read something else?

LMT DEFENSE
1600 EAST LEONARD COURT
ELDRIDGE, IA 52748. UNITED STATES

M203

LEWIS MACHINE & TOOL CO is their license name.
LMT DEFENSE is their trade name.

Although they'll likely accept either name...
If you have to pick one, license name is always preferred.

The above is how it's listed on a paper form (which traditionally includes the trade name).

Thanks to everyone who responded. My approval came back in 30 days.

I selected 'LMT DEFENSE Lewis Machine Tool & Co' from the drop down, "M203" for model, "40" for caliber, and "15" for length.
