The original discussion when 41F went into effect was two cards per serial number.
I’m fairly certain we’ve had a lot of people revert to the two per envelope method, including those using EForms. They simply include the cover sheets from multiple submissions in the same envelope. I don’t recall a single report of anybody gave an issue with filing this way. |

1 pair of fingerprint cards is all that is needed. You would just include all the applicable cover sheets with the 1 pair of FP cards when you mail them in.

I send two with each one. Cards cost very little so I'd rather cover my ass.

That first linked thread is archived now but the stamps I mentioned in that thread I just got back. I sent in one set of cards. No issues.
Edit: I assume e-forms would be no different. |
