The form HAS to be on one sheet, front and back. Hopefully I'll be back at the shop on Thursday if this damn Ice melts tomorrow, seems like the whole state of Kentucky is iced/ snowed in right now.
I've got some forms at the shop if you feel like driving up to pick them up. If you want to ask for Matt and tell Him that the forms are in Derek's Box. He'll hook you up. They are originals from ATF. I keep a few on hand for times like these. |

I don't have time to look for it at the moment but the ATF has a website where you can order all the forms you want and they will mail them to you.
