Do i have to have my aw form notarized if I have my DPS-3 form?

i guess i understand but if i am providing a DPS-3, then , why do i have to have it notarized? who would be lying, on an official form that they already have a copy of?
I guess my question more simply asked would be "who has filled out the AW form with a copy of the DPS3 without having it notarized" and having it been accepted and returned? if this is you please respond. if you had all the paperwork and still had it notarized please respond. if you did not, please respond |

The dps-414 form ITSELF has to be notarized. In addition to the "place notary seal here" area, there is even a box for their signature and other info UNDER where you put your signature. |
