Where in the Phoenix (west side) can I get my finger print cards made for my form 1 submittal?

Almost every UPS store does it. Call ahead to make sure the location closest to you does it. They do it cheap too around 7 bucks if I remember correctly.

Do yourself a favor and skip dealing with MCSO's B.S. and just go to an independent auto title agency. MCSO is cheaper as far as prints go but they don't have the actual cards from my experience....everything is digital.

I have had MCSO print me numerous times for my forms. There is an ATF branch in the first floor of the courthouse in downtown Phoenix and they can fingerprint you for a nominal fee literally 100 feet from the ATF window.

Call and see if MCSO has the ATF cards which they should, if not you will need to order them online and wait a week or so. Ask for Erin, if she still works there she can help you out. You will need passport photos as well, I just went to Alphagraphics for those, but I think Walgreens and the PO can do them now. If not doing the trust route, you will need to go through MCSO anyway as they are the CLEO of Maricopa County.

Double check, but you may need triplicates instead of dupes because I believe MCSO keeps one and the ATF gets two. Also, wait to affix your photos because MCSO ran mine through a copier and wrinkled the sh@t out of my photos.
