Where do you guy do your engraving?
What all do you put

Name, city state, caliber? or multi?
is the serial # required if it's already on the receiver

am I missing any thing?

If your building it (form 1) you need to have your name/trust name, and the city & state where it is being manufactured engraved on the gun.
1\16" high and .003" deep.
If your gun is marked multi, it will need a  caliber to.
The original manufactures SN is fine.

^^ there's no 1/16" minimum requirement for anything except serial number ^^

It does need to be marked somewhere on the gun.  It's pretty much always on the barrel though

You can believe what you want, but the law would disagree.

If the lower is marked "multi" and the upper/barrel doesn't have the caliber conspicuously placed, it has to be engraved.

The clearest explanation of that is #5 in ATF Ruling 2013-3.

Doubtful as most barrels are marked.

The exception to that is some 9mm barrels that I have seen.

Possible.

A lot of factory short barrels/uppers are marked, which satisfies the requirement. I have one I had to mark because I lost the barrel markings when I had a 6.8 barrel cut down from 16" to 12", and the lower is a "multi". I simply added it between the pivot pin ears with a Dremel.
