Do I fill in both of these- one for me and other for wife-
with Make- Model- Serial Number
And send in with all Trust information?

No. You don't need to fill in boxes 5 and 6 because you are likely not an FFL.

So, if I were to be filling the Form 1 out as a Corporation and I am an FFL, do I ned to fill in the FFL info?
