I have a couple q's and wanna make sure I do this right the first time.

On the form for item 13 it states:

I certify that I am the chief law enforcement officer of the organization named below having jurisdiction in the area of residence of

__________________________________________________________
                      (name of maker)

Does my name go here since I am the maker?

I would think so. This section is meant for the CLEO but he didnt fill it out. Would it be OK for me to do it?

Also the SBR faqs mentions a duplicate. Do I need to send them 2 forms? My CLEO only did the one. Would it be OK to make a copy of the form thats already filled out?

Im a little confused on what kinda photos Im supposed to use. The faqs thread says no proofs or random pics. I dont have a passport either so what kind of pic can I use? Can I just go to my courthouse and have them taken(and cut them down to 2x2) if not where?

Lastly the faqs thread mentioned filling out citizenship forms too? Where can I get these forms?

i believe you put your name there as you are the 'maker' of the SBR.

yes - you need 2 original forms - and need to have both signed off by the sherrif.

as for the citizen form - you can print one out from a link in the sbr faq at the top of the page.

as for the photos - you can get 'passport' photos at walmart for like 20 bucks.  you can probably find somewhere that is cheaper - but walmart was pretty convenient - so i just did it there.

Let your LEO fill out all of the information under #13  if you are filling out the form as a trust, email me and I can send you a sample

I always read about sending 2 forms, but I've had 3 SBRs approved as a corporation and have just sent a single form each time
