I have a 16" upper with the round handguard cap. How is this item affixed to the barrel? It appears that it is just pushed up against the rear of the front sight assembly, and the gas tube going through it helps it stay in place.

Should there be slight twisting play in the handguard cap?

The amount of "play" depends on who assembled the barrel parts.  But generally... no, there shouldn't be any.  However, it doesn't do any real damage to the rifle either.  You didn't say if there is "play" with the hand guards on or off.  If off, does it stop when you put them on?

There is a shoulder on the barrel that "pinches" the handguard cap between itself and the front sight body.
