My dealer said two cards with any number of forms is fine... I mailed off two F4's earlier this week with two completed fingerprint cards.

I'm going to do a couple of F1's so I was reading the stickied thread here on the new forms and there is a comment on the F1 as an individual:


Has anyone submitted one set for multiple forms yet, or do I get to be that "somebody?" 

These are my second and third NFA items, so I'm not exactly an expert. Thanks for your help!

As we specifically asked and received a response, we are submitting two with each Form 4 sent in until notified otherwise.

Awesome... that's what I was afraid of. So how do I rectify it? Just wait for one of the forms to bounce back and send the other two at that time?

I always bring a spare pair.  You never know how smoothly the fingerprinting will go.  One nearly always needs to be junked.

I'm not exactly sure what they will do. They may return one or both without action depending on the method and amount of payment submitted, or they may be sent on to WV for them to deal with it.

I'll just say be sure to keep us updated so I can update the sticky.

Thanks for being the guinea pig, or at least the first one I remember seeing say they only sent one set.

If you are "that guy," I'm right there with you.   Sent off 3 form 4's wednesday with only 2 sets of finger print cards (mine and wife) for our trust.   Guess we will be getting approval or denial pretty much the same time.  New paperwork requirements are onerous for sure.  I

That's pre-41F. Most everybody filed that way.

As a slight update, they cashed my check yesterday, so hopefully things are moving forward...

I call and spoke with examiner and Per them: 2 FP's, 2 Photos, per person on the trust. Plus Responsible party form, and 1 Trust per form 4 filed.
If you send more then one at a time then you will need to re-invent the wheel every time you file them.

So the consensus is (2) fingerprint cards per Responsible Person per Form 1/4 filed? Even if multiple Form 1/4's are mailed together?

What about the ORI box on the FD-258? Can the correct ORI info be handwritten in the box on a generic FD-258, or do they need to be ordered from ATF with the info stamped? Or leave the ORI blank and let them stamp it?

If by "them" you mean you or whoever is fingerprinting you, then yes. 

I've always used pre-stamped or typed them, but hand-written is accepted last I heard. All FD-258 cards are the same other than the ORI field.

"NFA" is fine, or "ATF Form 4".
