| Where do i get the revised form 1? Can i still send in an old form 1 from before 41P in regards to an individual? I have one I've been sitting on thats filled out, and signed by CLEO, but never sent it in. |

You can get them off atf.gov, or the "Work In Progress" sticky in this forum has links to all the new forms. A quick internet search for "ATF Form 1" gets you there as well.
