[ARCHIVED THREAD] - Trust - Back of Form 4

I've heard both ways that if going the Trust route, that you do or don't need to fill out sections 13 and 14 on the back of your Form 4. I'm filling out my first Form 4 for a surpressor and wanted to hear what you guys do, or your experiences. Thanks.

You need to fill out everything but 16 and 17 on the back of the Form 4.

I always have my customers fill it out....it is so simple, might as well do it to avoid a possible delay.

Thanks for the help guys, I actually just found this, found on page 5 of an FFL Newsletter by the ATF:

"Items 13, 14, 16, and 17 of the Form 4 do not require completion for a transfer to a corporation or other entity nor is the submission of ATF F 5330.20, Certification of Compliance with 18 U.S.C. 922(g)(5)(B), required."

I'd fill out 13 and 14. It takes all of five seconds to check no in all of the boxes.
