I have my form 4 filled out and signed by CLEO, picture, fingerprint card finished, BATFE form stating I am a US citizen all in duplicate, and $200 personal check. Anything Im missing?
I was also told to by the guy @ the class 3 dealer to send it to BATFE- NFA Branch, PO BOX 403269, Atlanta, GA 30384-3269. This is a diff address than listed on the form but he said he just used it several months ago and had no problem, 3 month wait. Let me know, Thanks |

Form 4's go to: Bureau of Alcohol, Tobacco, Firearms and Explosives National Firearms Act Branch P.O. Box 530298 Atlanta, GA 30353-0298 Newer Form 4's have this address on them (see http://www.atf.gov/forms/download/atf-f-5320-4.pdf). Form 4 with photos and fingerprint cards go in duplicate, but you only need to send 1 citizenship form |

On the Form 4 I linked above it says in box 1:
Submit with your application a check or money order for the appropriate amount made payable to the Bureau of Alcohol, Tobacco, Firearms and Explosives. Upon approval of this application, this office will acquire, affix and cancel the required "National Firearms Act" stamp for you. |
