Got my Cleo signature and fingerprints taken today. The PD in my home town printed off the fingerprint card but left most of the info fields blank. Hopefully someone can advise me what to put here.
The clerk signed the boxed labeled "Signature of Official Taking Fingerprints" and dated it. Below that is a box labeled "Employer and Address". Should that be the PD address,,,,or is that for "MY" employer and address??? thanx in advance! C |

I read elsewhere on here it's supposed to be for YOUR employer's address. I would put down your employer's info. That said, I guess they don't really look at this because lots of people have gotten approved putting their PD info in this box. |
| When I had my FBI-258 done last year,for my FL CCW, the officer had me write my employers info that box. His printed name, shield number, and department info were written on the reverse side of the card (his signature and shield number were placed in the respective box). I believe that box was left blank when I had a set done for my employment last spring (education). |

That would be for YOUR employer address. Why do they want that? Who the hell knows Lol. This is a generic FBI fingerprint card used for many different things. I think if you work at certain financial institutions they require you to get fingerprinted, and this form is used for that as well. Your employer info would be more useful in this instance. I don't think the FBI really looks at this in regards to NFA approval because people have put the wrong thing (PD info) in this box and have gotten approved. |

When I got my prints the LEO made me put my employer in the box. I didnt think that was correct so I called the ATF and they told me it was supposed to be the LEO employer. They want to know who printed you. I asked her what I should do and she told me to just put a mailing address label over my employer.

Do you really want them to have your employer's info? What if something was incorrect and they decided to contact the employer? Do you want your employer getting calls about you being fingerprinted by the ATF/FBI?
I don't think they ever look at it, though, for NFA. For my nine stamps, I have always put the LEO address. Never had a problem. |

My LEO checks ID's to be sure that the name and address matches what's written on the form before even taking the prints. He also fills in the PD's address after he signs. They also charge $20.00 a set. If NFA accepts "roll my own prints" I could save $40.00 for every stamp! |
