Sending in my first form 4 on Monday. Couple quick questions: who do I make the $200 check out to and what info needs to be on check? Also, do I need to include a copy of my C&R FFL with the other forms?

No extra info needs to be on it, but I always add "NFA Transfer Ser. No. XYZ123" so that the cancelled check becomes in effect a receipt. Unless it's required for state restrictions, I do not think you need to include your C&R. |
