Whenever sending off my form 1/4s I just include two copies of everything (Declaration of Trust, Certification of Trust, Form 1/4, Certification of Compliance).  When I first got into stamp collecting there were many debates on this forum about how many copies you needed and I figured more was better than not enough, so I just sent two of everything and had good results.

I am about to submit a couple form 1s (SBR and SBS) and was wondering if I had to double everything.  Do I really need to send in 4 copies of everything listed above?  How many copies of everything has everyone been sending lately?  I know I may have sent too many copies before, but I wanted to make sure nothing came back because of a lack of copies.

Also, if submitting two forms, do I need to include two separate checks, or will one check for $400 work?

Two of each form
Two print cards (if required)
One check
One copy of everything else

If you're stuffing multiple forms in one envelope you can be a risk-taker and only send one of each of the supporting documents and one check for the whole shebang, but stuff might end up seperated and one or more forms delayed.
