It's been quite some time since I've submitted an NFA application, and I can't remember how to fill-out the fingerprint cards.

Whose responsibility it is to complete the card?  Do I complete it; or does the Sheriff's office complete it?

What goes on the lines for "Your No.", "FBI No.", "Armed Forces No.", "Miscellaneous No."?

Thanks in advance for any help you can offer.

I can't recall what goes on the lines you mentioned but I had mine done at the sheriffs dept on a digital machine. The officer operating the machine entered my info, which printed on the card in the correct blocks along with my finger prints.

If you're using a sheriff's office to obtain prints I'd wait to see how they do it. The digital machine I had mine done on has a data base with all the cities, states and even countries in it. Hell it even had my country of birth - Morocco Africa in there! (Yeah I was impressed)

Thanks!  That sounds pretty cool.  I'd be a little surprised if my Sheriff's office has such a piece of equipment though.  It's a very small town.    It'll most likely be done by hand.

the officer who printed me had me fill in my name birthdate and signature. i filled the rest in at home

Keep in mind you don't have to use the sheriff of the county in which you live for prints. I didn't, however it helps when you work for the county that has the digital machine.

Just make a few calls and see if any LE office nearby has such a set up and what they charge to do finger print cards.

Do all the boxes need to be filled-in, or just the obvious - DOB, HT, WT, Hair, Eyes, SSN, etc.?

Our Sheriff basically let me put as much or as little as I wanted in the boxes.

For these "What goes on the lines for "Your No.", "FBI No.", "Armed Forces No.", "Miscellaneous No."?" I have always left them all blank.

Then you stick your hand in the scanner, then the other hand.

Then you tell it what card you are using (258's) and how many you want to print.  The ATF sent me 50, so I just printed all of them.

What can I say, I am an optimist my money will get here from the UK Super-Lotto soon

I had mine done at a security firm (local PD will not). The firm filled in most of the form. All I had to do was sign it.
