OK so I've got my:
Form 1 complete and signed (I love San Angelo)
2 sets of fingerprints cards
1 Passport Photo
1 Check for $200

Am I ready to mail it off?

You need two copies of the Form1 each with an original CLEO signature (unless going trust/llc), each with a passport photo, two fingerprint cards & one declaration of citizenship.

Oh yeah didn't know that, that's an easy thing here. Dropped the last one off Friday afternoon and picked it up today.

When filling out the certificate of compliance. Question two ask for the name of the person to certify my citizenship. Can a friend or co-worker just fill that out

And then what gets answered under question 4? Is the INS or admission number just a social security number?

No. That is where you put YOUR name if you are using a trust, LLC or corp (which goes in Question One). This is a self-certification form: No one except the applicant signs it. By doing so, you are certifying that you are a citizen, or a qualified non-citizen applicant.

Are you a U.S. citizen? If so, you skip from Question 3 to Question 7, and you do not answer Questions 4, 5 or 6.

If you aren't a U.S. citizen, you are an alien, so you must put your INS-issued alien number in Question 4. Then answer Question 5 and 6. If you are an alien without an INS-issued number, you are in the U.S. illegally, and should not be acquiring NFA items.
