So if I were going to sell a Form1'd SBR or MG, who whould I list on the form4 as the Manufacturer? Would it be myself, per the engraving, or would it be the original manufacturer?

You should list the person/entity which filed the Form 1 to make it.
The host gun's manufacturer should be listed in box 4h under additional description.
