Do i send in 2 Form 1's for 1 SBR?and if so will a copy work or will i have to fill another one out and sign? Thanks Mike

You must send everything in duplicate and use the ones they send you.

No copies ....

Both filled out and signed separately and separate photos ..... if you're going with an individual transfer ...

In other words - two originals ......
