I went shooting sporting clays today with some friends. Both had 500's, one a 20 ga. and one 12. Every once in a while, when they would pump for the second shot, the live round from the  tube would fall out. It looked to me like it was flying out the ejection port, right behind the empty hull.
Have any of you seen this happen? Is there a fix. It happened to each of them about 10- 15 times on a 125 round course. One guy started loading extra rounds in the tube and it stopped doing it.

He just took it apart and cleaned it for the first time in about 5 years, so maybe he reassembled it wrong.

The other guy I don't really know, so I have no info on his gun. It's just weird they both were doing the same thing.

I had this happen with my Winchester 1300 once and have never had it happen again not sure what could be causing this but interested to find out.

I have a 500 in 20 ga and it does it sometimes but not too often. I think it happens when you try and rack the next round too fast. I know my gun is put together correctly so that isn't the problem. I think it is just how the Mossbergs work sometimes.

They were shooting Remington gun clubs mainly.

My 500 would drop the next round out the bottom loading port.  Sent it back to the factory and haven't had the problem since.  No idea what the solution was.

I've read a few cases where the interrupter was causing shells to drop out the bottom or extra shells trying to load and I want to say it was with older models.  The only quirk I've had with my 500 is sometimes it does not want to cycle the bolt back.  I'm not sure, however, if its the bolt that's catching or if its the slide.
