What is the purpose of the hook on the end of the folding stock on some Spas-12 shotguns? So far as I can tell it exists solely to be a major pain in the ass. It looks like it would make using the sights impossible while the stock is folded, and it'd be useless pressed against your shoulder as a stock. So why is it there, and why do shotguns that have it cost more than those that don't?

i believe its so that you can stick your arm through it and use/steady the shotgun with one hand.

It is a carry hook ONLY, and is ONLY functional when the stock is folded (but not completely folded flat...you have to keep the small butt plate/shoulder plate at the end still sticking up, so that the hook is perpendicular to the shotgun).  I really enjoyed my SPAS-12 (since sold), it was a great shotgun.

You are correct about the sight-line being covered when the stock is folded.  However, I don't think that you would want to try to use the sights when the stock is folded...say hello to a broken nose or worse

As far as being worth more with the Carry Hook included, that's just because to a collector it is considered a complete factory package with all accessories.  The Carry Hook twists out if you don't want it there.  That is why many no longer have them included.  The original owners took them off and lost them.
