Nice looking pistola you got there.
It might be the extractor, I had a used Springer I bought that was doing this occasionally, I replaced the extractor and put an extended ejector in it and now it launches the casings perfect every time. It also could be your grip on the pistol, if you happen to limp wrist it (which I've done myself after a bunch of shooting) it can cause casings to come back at you. It looks to me like your MSH is sticking out the bottom of the frame a few milometers, almost looks like the frame was cut high or the MSH is longer than it should be. |

Likely the extractor or the ejector. I was surprised to find the ejector was not machined well and would bind on a different slide I would put on it. Very nice 1911 though! At first mine had all sorts of problems with failures to extract and then I tuned the extractor and nothing since. I too am surprised by how accurate these are. |
