I've had this thing for about a year and probably have 5-600 rnds through it. Every now and then it will jam the empty case back into the mag and prevent extraction and jam the slide open. It has hapened on Taurus, Kimber and Wilson mags. What could be causing this, becasue it is a pain to pry out of there and drop the mag?

Mine had the same issue.  Replaced the extractor and all is well now.

Is this what the malfunction looks like?

Put a Wilson Bulletproof one in. DO NOT go cheap, spend the money initially and save yourself grief and wasted time. Have it set correctly by someone who knows what they are doing or do it yourself, it's not that hard and I believe there is a sticky on it. All you really have to do is make sure the lower edge is beveled correctly and that it is tensioned correctly to hold a cartridge correctly when it is inserted under the extractor claw with the slide removed from the frame.

Hey, did either of you that had extractor problems also have a problem with brass ejecting straight back? I too have a PT1911 that's been doing that, and almost every casing has a large dent on the case mouth about 90 degrees from the extractor mark. The picture (above) makes me think my problem is extractor-related, even though the tension seems correct.

The problem can be resolved by exercising your hand.
While you may think your grip is solid, I have had the same problem. I purchased a Grip Master from Prohands.net and in two weeks the problem was solved!
Recoil operated pistols demand a stable platform (your hand and wrist) without it the EXACT problem you describe will occur.

Lighter recoil spring on the PT1911 + weak wrists...

I have not any issue with mine, of course I have a grip that is like a table-mounted vice (solid and stable).
