So I have been carrying around a SpecOps spare mag pouch for a while now. I liked it because I could carry it vertically or horizontally. I am thinking about switching to a IWB set up, but I dont mind sticking with OWB. But its for a single stack 1911 Wilson mag. And would prefer either a horizontal draw style or one with a cant possibly but open to anything thanks!

+2 And not worth the discomfort in my opinion.

Consider carrying your spare magazine horizontally in a nylon multi-tool pouch with a Velcro closure; it stays tight to your body and if it's spotted; people will mistake it for a multi-tool.

It's not the most tactical way to carry your spare magazine but it's comfortable and well concealed which means you'll always carry it.

The downside is that it takes up so much room on your belt that one is all that you can carry that way.

Another +1 here for IWB mag carry being really uncomfortable.

I wouldn't recommend a Fobus holster, but their mag pouches are GTG.  This pic shows their double mag pouch next to a single Blackhawk pouch.

I agree with E5K.  I have had a Fobus 9mm/light combo pouch for years and it has held up and I've never lost a mag.  Conceals under a t-shirt!

I agree with the vertical IWB mags being uncomfortable but I have been playing around with mags stuck inside my belt and with the right forward cant I think they could be really comfortable. I haven't been able to find anything like that available. I am thinking about asking someone to make me something custom but it would make for a pretty expensive experiment. Something like this I think could work really well with an approximately 45 degree forward cant:
