I had some problems with one of my G builds, so yesterday I took it out and I brought the camera with for pics.

Here we go,

Insert magazine, and I get a failure to strip a round off, rack it again and it will chamber, but I have to manually slam the bolt carrier in place

after firing, I get a failure to eject,

Try it again and same result

So I swapped out the Bolt and Carrier form my other G that works great, and problem solved, the rifle runs 100%

So I try the in question Bolt and Carrier in the other G that has never had a problem and it works 100%

So this has me stumped

I kinda think it is in the serial number

What do you think?

Probably something to do with a headspace issue.

How does your bolt carrier that goes with that kit slide on the rails w/ no FCG in?

Not sure what receiver you are using but I had a similiar problem with my Romy G I built on an OOW receiver.

I think when I hammered in the new rivot for the US gas piston I bent the carrier just behind the threads. My bolt carrier was really rubbing on my rear gas block.

I swapped out the bolt carrier with the one on my Wasr10 and vice a versa. That fixed the problem. The WASR I have was a real beater with a canted gas block. Switching the carrier with that one did the trick.

I would just switch them and be done with it.

I think you are right.  It is the serial number.  That rifle is cursed!

Is your chamber possibly very dirty?  A friend had a similar problem with a Chinese AK and it was fixed with lots of chamber cleaning...

sounds like an extractor problem, i simulated your fisrt pic with my rifle and a dummy round and slowly chambered the round making it look just like your pic. With that kind of carrier gap the extractor did not go over the rim of the case and was  hitting the bottom of the case, this is why you had to hit the carrier with your hand to push the extractor over the case rim. It could also be why your not extracting the cases too, if it doesnt extract the case they just lay in their to get pushed around by the bolt again( ie your 5 other pics ) Check the extractor spring or just get a new one and clean out the cavity. And remember those are just upside down 9's.

Well I finally got around to tinkering with this rifle.
I tore it all apart, and I found a burr on the outter edge of the gas piston.
When the rifle was closing the gas piston was getting hung up on the gas block.
A few passes with a file and some 1500 grit and she works like a champ
