Did all of the Russian and Bulgarian AK-74's come with the side mount scope rail setup? I know the AK-47's didn't but all of the pictures that I'm looking at right now show AK-74's with a scope rail mount.

No they didn't. I beleive AKS-74n and AK-74n had rails, AKS-74 and AK-74 did not.
