Is it chambering a round?

Is the hammer cocking?  Do you get a click when you pull the trigger the second time?

Are you seating the stock solidly in your shoulder?  If the entire rifle recoils it's possible to "limp shoulder" and interrupt the feed cycle.

Has there been any other modifications to the rifle?

Did the rig full stoke, and you are sure that the bolt was locked up?

If yes, then break down would not fire for us, hence dead trigger, or when you went to pull the trigger, you could hear the hammer come down to strike the FP, but the primer would not ignite.

If it passes the function check, it might be a weak trigger spring.

Are you getting a dent on the second primer?

i would push the bolt release to would fire the first bullet and chamber the next round. i would to try to pull the trigger and it would feel like the safety was engaged it wouldn't move   so when i tried to pull the charging handle back the bolt was tough to pull back. the rest of the day i put in one bullet at time and i had to put it safety first then into fire to shoot the bullet

Sounds like you have hammer follow malfunction going on.  Trigger may be out of adjustment or have some other issue.

Try the standard function check (for safe and semi), and report what happens.  One extra step of note not described in that guide page is that when changing safety from safe to fire(semi), the hammer should not drop.
