Here are the specs:

-Brand new RRA .223 Wylde upper with 20" bull barrel
-Spikes lower (functions flawlessly with Stag 16" M4orgery upper)
-Ammo: Hand loads; remington peters cases with sierra 63 grain bthp (these also function flawlessly in the Stag 16")
-1 new 30rd P-mag
-2 used 20rd Colt mags

Here's the problem:

If the mag is loaded with more than 5 rounds (this happens with both Colts and the P-mag), the rifle will fire, eject the spent casing, but it will not load the following round.

If the mag is loaded with 5 or fewer rounds, everything functions correctly.

Any thoughts on what might be causing this?

Specs on the loads, and have you check for signs of them going over the top in your rifle, or just loaded too light to begin with?

Is the BCG dry? My FTF's disappeared when I introduced them to CLP.

Whats your case trim length, COL, what bullets, powder and primers are being used?

Got it figured out.

The case was catching on the magazine and would not allow it to feed properly.  It's a little hard to describe...sorry.  Just went through and filed off that little part of a junk magazine to test to see if that was the issue.  Sure enough, worked like a charm

Not so much.  Problems are back.

From the site,
data.hodgdon.com/main_menu.asp

69 GR. SIE HPBT
IMR 4198   Bullet Dia .224"
OAL 2.235"

Min
18.3   2706   46,300 PSI

Max
19.5   2818   52,300 PSI

With the above data, you are over the top, and even worse with the use of a mag primer. Plus, you may even be flirting with bullet embedment with the longer OAL (guessing that you have at least confirm the needed OAL for kissing and the reason that you are longer).
