Called a "slam-fire" technically, suspect the problem was combination of the AR15's free-floating firing pin and a not-properly seated primer on the Privi ammo.

This usually occurs when there is a problem with the Fire Control Group (FCG).  A weak or improperly installed disconnector spring would be the first thing to consider.  Be sure the larger end of the disco spring is on the bottom.  Next up would be the disconnector, trigger and hammer.
