I'm sorry didn't specify the problem in the title, but I really do not know what it was.  

During my first year at my first unit, I had an M16 that would fire only if the round was chambered by racking the charging handle.  It would go something like this:

a) Fire, unlock, extract, eject, cock, feed, chamber, lock, and click-failure to fire
b) Rack charging handle, fire
c) Repeat a and b until qualified

The armorer couldn't identify the problem for whatever reason so after a couple months I ended up with a new weapon.  

I still wonder to this day what the problem was.

Really hard to say without knowing all the specifics of the rifle what the problem was (what was happening internally to cause the problem).

Need to remember that this can mean simple skilled from parts changing alone (must have the manuals open or they get lost), to the Smiths that build the match rifles for the shooting teams/ sniper rifles.

Although the latter group are "Gun Smithing Gods", the same can not be said for some of the others/entry level personal (parts changers); having seen more than a few of what are called armors that I wouldn't even let them change the pistol grip on one of my personal units for fear they could even screw that up.

He was definitely not one of them.

This was our supply guy who had been to Armorer's School (US Army), and had the keys to the arms room; which was a burden to him.

I really wish I could elaborate, but at the time, I knew nothing more than what I was taught in basic training.  All I remember about the rifle is that it was a FN M16A2.

+1 to Dano  -- no way to know now.

Only out of curiosity-- did the unfired rounds have firing pin indents? light strikes?
Any possibility of dirty firing pin channel or bent firing pin? firing pin length?

This was years ago and I can't remember very well.  I want to say yes, but I am not certain; I'd hate to through everyone in the wrong course.  If that was the case, why would it fire when manually operating the bolt?

No, sir.  No such thing on a weekly over-cleaned Army weapon.

I should mention that this weapon was pretty worn.

If I had this rifle in front of me, I would do the entire trigger/hammer/disconnector check. If the hammer/trigger/disconnector are not working correctly (springs in wrong, etc), you could get this malf.

Since it is history and there really is no way to tell now, it is curious but also gone.

I only mention this because I had a similar problem. brand new rifle after around 300 to 400 rounds. Rifle was cleaned after every session. I had the same thing fire some rounds?? then click no boom! rack manually and then it would fire a few then click. repeat.. I cleaned more aggressivly in the firing pin channel?? or rounds fired broke it in furhter??? I think it was cleaning error on my part! Probably not your problem.
