First time this ever happened and hope you guys can offer some insight. I have a A1 upper I was given years ago by a gunsmith friend. He swapped it out for a flattop on a customers gun and the customer gave it to him. It is marked PM if I recall correctly.
It worked great for 3-4 years (only about 200-300 rounds) and all of a sudden the FA (teardrop) popped out at the range. I recovered it and went home and reinstalled it. It later popped out again while functioning the action. It would not stay in well at all after that so I ordered another FA unit (all new parts) and installed it. That one stayed in for a while and while showing the gun to a friend and cycling it the FA popped out again. How can I remedy this? Any easy fix and what might it be? If it cannot be easily fixed where can I buy a plug for the FA hole? It is not one of my primary AR's and simply a fun range gun but I want it to work well....Thanks.....<><....:)

i assume you are talking about the forward assist?
is the roll pin still in place when it pops out? if so it was installed incorrectly,

yea if the roll pin is popping out and you tried different kits the hole is probably too big, you could probably get a roll pin that is a bit bigger form a hardware store,

Without FA the gun will work just fine.

I wouldn't try to put in a plug.  The plugs (at least the ones I've used) are held in place by the same pin as the FA.  So, if the FA is falling out, the plug won't be secure either.

the FA hole location is off, not allowing the pin to retain the paw on the FA.
to fix open up the hole to .125 and use a trigger guard roll pin.

Thanks to all and especially the above. This may work out well for all involved. I have been contacted by a member who has been looking foe this particular make/model/manufacture upper receiver and since I have no particular attachment to it we may possibly work out a trade. If not I will use it after fixing it. However I may use a pin a bit smaller than thge trigger guard .125 first to see if it works.
I can always go larger if it does not. I'll first try just a few thousandts larger than the original pin that is in it....<><...:)

that most likely will work. That hole should be an .093  + or -  so opening it up by .032 isn't much. and trigger guard roll pins are readily available and it works.

Also look at 2.5 and 3.0 mm pins as options, start small and work up, because if hole is drilled too big you will not get the pin installed past the fa slot

So have someone measure clearance before you drill holes out bigger and ruin upper because pin will be too big diameter
