I'm assembling a new pre-ban upper and plan to put a Panther/Phantom flash hider on it.  Is it necessary to put a crush washer on with it?  If so, which way does the crush washer go on the barrel?  How tight must the flash hider be screwed on with a crush washer?

Concave end of the crush washer towards the flash suppressor. Tighten to 15-20 ft/lbs.

Does the Phantom require indexing?

I use the lock washer that comes with the Phantom. No, they do not require indexing, at least the new five slot ones do not. The four slot Phantom, index so the slots are at 2-4-8-10 o'clock.

So the gases escaping from the slots produce a downward force and the side forces cancel each other out. If the slots were aligned differently you could have a rifle that tends to pull to the side or has increased muzzle climb when you fire.

One could also position it at 1-5-7-11 but doing it at 2-4-8-10 directs the gas more to the sides and kicks up less dust on the ground when prone. As long as the slots are diametrically opposed to each other, even in pairs, they will always cancel each other out assuming the F/H is machined properly.

There's no thrust, all sides are open. Besides, if the slots are 2-4-8-10 the push, if it existed would be diagonal.
