I am contemplating making a shaved front sight gas block and using a set screw to hold it in place.

Is a single set screw strong enough?

I'm thinking the gas pressure that is sufficient to operate the bolt and feed the next round will also try to push the gas block off the end of the barrel.

Has anyone ever had a gas block held on with set screws come loose?

I have used two or three set screws, never one, to hold the gas block in place.  Never had one come loose.

I  would recommend 2 or 3 setscrews to secure your gasblock.  It also helps to mill (or file) a small flat spot on the barrel where the setscrews will make contact.  The setscrew(s) deform the barrel slightly and make future gasblock removal difficult over the burr created by the screws.

One, two or three; I'd use a little blue or white Locktite for added insurance.
