Had a problem today with one 30 rd mag with a green follower (it looks like a standard USGI type mag with Kay Industries on the floorplate).  It would not lock back when empty when fired with my 20in Bushmaster flattop.  It locked back when empty with my 14.5in Bushmaster carbine.  I tried it several times and this occurred consistently.

What should I look for if this is a mag problem?

Sounds like the malfunction could be caused by the interaction of your ammo & rifle.

This only happened with one particular magazine.  The 20in Bushmaster operated normally with the other mags.  That's why I think it is a problem with the mag.

One of my new mags from CMMG does that as well.  It even has a magpull follower.

Had the same problem with my Bushy. The mags worked fine in my Colt but failed 50% of the time to stay back on an empty mag in my Bushy. Also, all mags failed to drop free in the Bushy. Never did solve the problem but I don't believe it was a mag issue.

IMPOSSIBLE!!!!  magpull followers solve all problems  Seriously, it is a mag problem and the cause is that the spring does not put enough force on the rear of the mag where the follower trips the bolt catch.  Sounds like you need one of those $6 springs I've been hearing about.
