Messed-up primer, probably went in sideways. Best way to describe that is "shit happens".

yup. with hundreds of millions of rounds loaded, it's gotta happen just by the numbers.

good thing you eyeballed it first!

When production levels are pushed near the max, it isn't unusual for a few mistakes to get through the QC process.

The primer was seated sideways. Discard the round and carry on.
