I was firing Federal Red Box (FC head stamp) in my 16" Spikes AR. The gun only has about 1000 rounds through it and is very reliable.

A round failed to extract and was very difficult to remove because the bolt would hardly rotate in the carrier. When we finally got it out a piece of the case neck was missing. I'm not real sure what happened and it had somehow jammed up the action making the bolt very hard to rotate.  I have fired 500 rouds of this ammo and it has been great - but no more just pulling them out of the box and stuffing them in the magazine - This cartridge must have had a noticeable neck defect before chambering.

I bought 2 boxes of Remington at WallMart and later read a warning on the ammo section about their quality.  Went to check them and found one round with a cut neck.

Never had a problem with FCs but after that I check them all before loading the mags.

You should do a visual everytime you load your magazines. Better safe than sorry.
