[ARCHIVED THREAD] - Failure to Extract Q3131

I recently bought 300 rounds of winchester q3131 year 1999. With 154 rounds fired so far I have had one failure to fire and today one failure to extract. Here is a picture of the case.

What happened here? Also, the last two rounds after this one doublefed.

Picture is a bit fuzzie... Think you could get a better one?

I can't really make it out well, but the way that case is messed up doesn't make a lot of sense from what I can see...

Was it an actual failure to extrct it from the chamber or did the case just get stuck in the receiver? By the looks of it I would guess it just stuck in the receiver and would clean the extracter and check the extracter spring if thats what happened.
