I have done some research and have been unable to find the answers, so I turn to the board.

What is the evolution / reasons for the colt bolt?  What came first, wide band, narrow band, etc?  What was the thought process that made them go from one band type to the other, and finally settle on the current "medium" band standard that we have now?  Was one type more / less reliable?

Also, when did they start to incorporate C-158 into the material of the bolt?  Was it not until the M4 carbine came out, or did it happen earlier?  I know the early bolts were made out of 8620 steel, and the M16 rifle length gas system was not too hard on bolts, so there was no real reason to switch to a stronger material under carbines started to become more prevalent.  

Anyone have a rough date range as to when the switch in metals was made though?

The thin bolt was early, right? The wide band bolt replaced it due to more surface area handling wear and tear better. The medium I do not know, but assume it may be more efficient (less drag) than the wide band.

C-158 has been used going back to at least 1970 by the blueprint below. You can make out for Note 1 (top left) it say Carpenter 158, and the date 27 APR 70 in the table (bottom right).

I am very interested in why so many changes but other day than the knowledge of the first 5-6 types/changes I'm stumped. I'd love to know why all the changes but it has to do with function vs cost I would bet.
