Is it actually trying to feed two rounds at once?  Could it be that the fired round didn't extract and then a second round was fed on top of the round that didn't extract?

Once you're ruled out mag, it's just about always gassing or extractor. Usually gassing, over or under.

If barrel port is right diameter, gas block installed correctly, and port and tube not plugged, can be modified with buffer and/or spring.

Barrel was changed to correct gas port size. It fires one round pulls it out of the chambe and kicks another one in the chamber. The empty ends up on top of the unfired round(which is only 25% in the chamber). Baffled. If I load a mag it cycles through using the charging handle with no problem.

Sounds like it's short stroking. You didn't drill the barrel did you?

Have you upgraded the extractor spring?

Are you running the gun wet?

What kind of gas block?

What buffer tube, spring and buffer you using?

Need details to help any if possible!

I don't mean to hijack your thread but I'm having the same issue and my other thread was deleted.

First off i have not made any mods or swaps that would make a SBR.

I have a new double star 7.5 upper and semi auto BCG.

I am getting the same double feed issue, one empty and one live round.

I've swapped buffer spring and buffer along with a full auto BCG, from my psa 16" carbine.

As far as mags, regardless of parts, one round and no mag, shell will not extract in the pistol. Again in the rifle works fine.

All parts from both guns work in the rifle flawless.
All parts in the pistol i get the same result.

I'll try and post a pic of the jam in my next post.

I would try an extractor spring upgrade.

Mine is a piston upper with a proprietary bcg. I've sent it back to the manufacturer twice. They said it worked fine after the initial barrel swap. It was overagassing prior. I run a Spikes pistol buffer tube/ buffer/ spring mated to a Seekins precision lower.

I had the same type of double feed issues you describe.  I upgraded the extractor spring setup to a BCM set http://www.bravocompanyusa.com/BCM-Extractor-Spring-Uprade-Kit-p/bcm%20extractor%20spring%20%203pack.htm Three sets for $12, you replace the extractor spring with heavy duty one, a new spring insert, and a small o-ring, it worked great.  No more double feeds.  Good Luck.

Change the extractor spring. You can get Wolff brand springs from Midway for $5.00. If that doesn't work turn your gas up on your piston and make sure the gun is wet (oiled well).
