JP/VTac style rail with QD sockets in the barrel nut? I know I've seen it, just don't remember where.

No, this one was much closer to the JP style. I've looked at Carbon Arms and Firebird Precision. They use the same style barrel nut, but no QD sockets.

is this a carbon fiber tube you are talking about? AP Customs does them with holes to accomodate the noveske TRX swivel mount. evidently this is a custom request

they also look to have come out with their own sling swivel mount:
