Has anyone ever had an issue with the cutout on the upper receiver that holds the charging handle, as in it being too worn to hold the charging handle so when you shoot every once in a while the latch pops out and hits you in the nose.

I had a latch that was out of spec not long ago and didn't lock into the groove in the upper and therefore allowing the handle to slide3 back.  I filed the latch until it reliably locked in place.  A new handle probably would have been an easier fix, but I figured I didn't have anything to lose by trying to fix it myself.

I've seen some very old M4s & M16s and never saw/heard of the CH and/or upper wearing so much that it wouldn't stay latched.

On one of my Anderson uppers I accidentally ripped the CH back while the bolt was locked back (I thought it was forward )
I ended up rolling the corner of aluminum on the upper that holds the CH latch in place. As in a decent amount of aluminum was removed. It still holds in place though, I just made sure to pick my stiffer-latched bushy CH in it just incase.

No not yet. It is just getting worse. pops out while firing hits me in the nose. Kinda annoying.
