I bought my first AR in June of '12 so I'm new to the platform but I've been shooting for 25 years.  In the past I've owned AK, SKS, and M1 rifles.  My AR had its first malfunction today and I've never seen anything quite like it.  The rifle: Bravo Company 16" mid-length upper, BCM BCG, and Gunfighter CH on a BCM lower.  The only "building" I did was putting the upper and lower halves together.  So, it's a decent quality rifle.  The rifle has 730 rounds through it, and until today it has been flawless.

I was shooting PMC XTAC XP193 5.56mm 55gr ammo, loading 10 rounds at a time into a brand-new Colt 20 rd aluminum magazine from the bench.  The rifle started the day clean and well-lubed.  The malfunction happened on the 16th shot of the day.  When I fired the round I thought I had some sort of double feed, but when I removed the mag to clear the rifle I found this:

Yes, the fired casing was extracted from the chamber and somehow ended up back INSIDE the magazine, fully beneath the feed lips, where is was subsequently smashed into the front of the magazine by the bolt as it came back forward.  As you can see in the first photo, the mouth of the case was severely deformed when it was smashed into the front of the mag.  I removed the offending casing, reinserted the mag, and fired another 70 rounds from the rifle without any more problems.

I don't think this means there is something wrong with my rifle.  If my gun had a major problem I think I'd see more evidence than this one incident.  But still... how the hell did this happen?  Anybody ever see this happen before?  It just doesn't look like any malfunction I've ever experienced before.  If I'm wrong and there is something I need to look at on my rifle, please let me know.  Thanks to all for their time.

IMHO, I just don't think that is possible. of course I am sure I will be told I am wrong. Did you load an empty case?

I was about to type a strong denial that I would ever do something that stupid... but then again... I WAS using an empty case to adjust my rear sight and it was sitting on the bench right next to me.  I suppose it's possible that I jammed it into the mag without noticing (!) during a break.  I, too, fail to see how an empty getting jammed that way would be possible.

Feel free to fine me 10,000 internets for my dumbassery.

This, impossible otherwise

If you didnt accidentally load an empty case in the mag, that is a million in one shot, maybe a billion in one.

I hate to say impossible but this is really close, I'm not seeing this being possible.
I'd bet you loaded a empty but I guess it not impossible.

It is not possible without loading an empty case.

That makes sense. And apparently a lot of people don't actually read through threads before posting lol.

Buy a lottery ticket maybe this is good luck... but really you probably caused this one.  Did you happen to notice if a case ejected when the jam happened?

Either you can't find that rear sight adjusting case, or you've been letting Italians monkey w/ your AR:

Jam is what you put on bread.  You had a malfunction.  And it looks like you loaded an empty instead.

In any case it was an ammo malfunction. Most all AR malfunctions are going to be either ammo or mag.  If you don't cheap out on either you won't ever have a problem with a decent AR.

I'm going with this.

Not possible for the case to end up back in the mag.

IMO you probably loaded spent brass by accident.
